Attendance and Cell Phone Policy Acknowledgment Form
I acknowledge I have read and fully understand the attendance and cell phone policy for Principle LTC. I understand the lack of following this policy may result in disciplinary action up to and including termination.
• Attendance Policy:Agency staff are required to have good attendance. This means being at work and ready to work at the scheduled starting time each day. Absenteeism and tardiness places a burden on other employees and on our clients. Temporary employees are bound to the client’s attendance policies as well as those of Favorite and must therefore be aware of and responsible for following both policies. Documentation regarding reasons for attendance issues may be required. Transportation or childcare issues are not considered an excuse for attendance issues. Cancellations, “No Call No Shows”, tardiness, leaving a shift early, and misuse of break time are all considered attendance issues and will result in disciplinary action up to and including termination.
• Electronic Policy:The use of personal electronic communications devices, including but not limited to, cell phones, smart phones, camera phones, texting devices, PDAs and the like are prohibited during the interaction with and/or during the delivery of resident care services in non-designated areas of the facility.
The term “use” includes operation of any device function, including answering the telephone, sending and receipt of digital messages including email and text, searching the internet, taking, or viewing photographs, playing games or any other possible use of a function or application available on the device.
In the event of a pending personal or family emergency which necessitates the use of an electronic communication device by an employee, the employee should notify their supervisor of the emergency and seek permission if possible, given the current resident care requirements to go to a designated area to respond to the emergency.
Employees with company issues electronic devices may use these devices in non-designated area for business purposes only or those areas specifically designated for the employee, this will include compelling business emergencies requiring immediate attention or assistance.
Employees are permitted to have or carry personal Eltronic Devices on their person (e.g., in a pocket or a personal handbag) while on duty. However, employees may only use personal electronic devices in designated areas. Designated areas for purposes of this policy shall include, but not be limited to, break rooms, administrative offices, and other non-patient areas. Nurse’s stations shall NOT be considered a designated area where use of personal electronic devices is permitted. For all individuals that desire to secure personal belongings you may story personal electronic communications devices in the designated employee premises of the building.
An employee who is actively using (texting, talking, emails, etc.) an electronic communication device during the interaction with and/or during the delivery of resident care services in non-designated areas of the facility will result in disciplinary action up to and including termination of employment.
The audio and/or video recording of any individual in the company is strictly prohibited unless approved by management and with the express consent of the employee. This applies to the entire company, working and non-working areas. Violation of this prohibition will result in the employee’s termination.
